Please read through each page carefully to ensure that all information is provided accurately. A Level 2 background check will be conducted before a team is assigned to you.  It involves a national fingerprint-based check and a comprehensive Florida Department of Law Enforcement (FDLE) and FBI checks.

This background screening requires a person to enter an accurate US Address.

• ALL COACHES FOR Fall 2025 WILL BE REQUIRED TO ATTEND A CLINIC PRIOR TO THE START OF THE SEASON. THERE WILL BE AT LEAST TWO DIFFERENT DATES OFFERED FOR THE CLINIC. 
• All coaches must provide a Concussion Management certification.  https://nfhslearn.com/
• As part of viewing the video, you will take a short test and then be provided a certificate of completion. These certificates are valid for 1 year.
